24 month	Immunization Catch Up
3 year	Immunization Catch Up
4 year	DTap, IPV, MMR, Varicella
5 year	Immunization Catch Up
11 year	Meningococcal, Tdap, HPV
16 year	Meningococcal, Trumenba
FALL	Influenza (>6 months old), COVID (>6 months old) and infant RSV vaccines annually

We recognize that the choice to vaccinate may be very emotional for some parents, and that it is your right to make decisions regarding the vaccination of your child. However, we believe in the safety and effectiveness of vaccines to prevent serious illnesses and save lives.
